State Medicaid Budgets Will Decline by $665 Billion Under New Federal Law
This Top Stories report summarizes a recent Stateline article outlining the projected $665 billion reduction in state Medicaid funding and the key policy changes driving coverage and reimbursement impacts.
A March 4 analysis found that the 2025 reconciliation law (“One Big Beautiful Bill Act“) will reduce state Medicaid budgets by a cumulative $665 billion over the next decade, driven by $911 billion in federal Medicaid spending cuts.
Key 2026 provisions already taking effect include new restrictions on provider taxes; limiting states’ primary mechanism for sustaining Medicaid funding – with at least seven states needing to revise provider tax waivers by April 1, 2026. Also, the sunset of enhanced ACA expansion FMAP incentives and more frequent eligibility redeterminations.
The multiplier effect on total program spending is severe: every $100 million reduction in state Medicaid spending translates to a $286 million reduction in total program spending, posing existential financial risk to rural hospitals and safety-net providers heavily dependent on Medicaid revenues.
State Medicaid agencies and managed care plans should model the downstream coverage contraction and prepare for increased uncompensated care burdens.
