CMS Final Rule Eliminates Fax and Mail for Claims Attachments

CMS has finalized a rule that replaces fax and mailed documentation with standardized electronic transactions for claims attachments and electronic signatures. The change is expected to reduce administrative burden across the healthcare system and generate approximately $782 million in annual savings by improving how clinical documentation is submitted between providers and health plans.
The rule applies to HIPAA-covered entities and introduces a consistent framework for exchanging supporting documentation such as medical records and lab results. It takes effect May 26, 2026, with full compliance required by May 26, 2028.
